ADMS Enabled Outpatient Education & Management Program as a Seamless Extension to a Regional Pediatric Diabetes Center


Population

Children ages 2 – 18 in South Texas with Type 1 Diabetes


Results
  • Average A1c reduction, the gold standard for measuring a patient's control of their diabetes, of one-half point in a subset of the overall population currently involved in a randomized, controlled trial. Mid-point results are planned for publication in Q2 '2009 with final results to be reported in early 2010.
  • High participant satisfaction is consistently reported

  • Proving that a cost effective and highly scalable technology driven program can be an effective program on its own or as an adjunct service to labor intensive call center disease management programs
  • Driscoll Children's Health Plan now offers this program as a covered benefit to its members enrolled in the State of Texas Medicaid Programs.

Description

Automated education & management services provided as a completely turn key program including participant recruitment, enrollment, fulfillment, active monitoring & education, analysis and feedback to a patient-centric team of supporters (aka - social network) including the diabetes center staff, family, school nurses and friends. There are no financial incentives included in this program.

Trial Sponsor

Funding for the current randomized, controlled clinical trial provided by the USDA Distance Learning and Telemedicine competitive grant program and sustaining support from Driscoll Children's Hospital and various health plan sponsors.

Technology

ADMS configured to emphasize frequent pattern management of Self Management Blood Glucose (SMBG), simplifed and accurate patient-provider communication around SMBG and streamlined remote patient monitoring for patients in the inpatient setting who are newly diagnosed with type 1 diabetes. Patient families are issued a care kit including a GlucoMON wireless blood sugar transmission device, registered blood glucose meters, optional data only cell phones for two-way text messaging / email, daily or weekly receipt of Diabetech's Day over Day blood sugar report and optional real-time copies of blood sugar from school, home or hospital sent to one or more destinations including cell phones and email.
