The type 1 new onset program and the information on this page is addressed toward physicians, CDEs and hospitals as a better way to manage the flow of information from the inpatient setting immediately following diagnosis, to nurse, to physician and continuing as a seamless transition to the outpatient setting.
Focus on efficiency
This program has been designed to support the new onset family as they learn to care for their child with diabetes while simultaneously streamlining the flow of information between the patient, the floor nurse and the patient's attending physician. By making sure that the data in the glucose meter is accurate, automated tabulation and distribution of blood sugar data allows the nurse and physician to spend their time more efficiently with a focus on treating the patient. It also means that patients get to learn about pattern management as a technique for taking control of their diabetes even before they leave the hospital.
How it works
Care Kits are pre-packed and pre-configured for your practice so that they are ready to go simply upon opening and use. The mygluco support team handles ALL technical data management and support questions for the patient.
Step 1 - With the initial visit on the hospital floor or in the office (depending on your protocol), the diabetes team breaks the seal on the patients T1 New Onset Care Kit and instructs the patient how to use their blood glucose meter. The patient is also introduced to the GlucoMON® from inside the Care Kit and in about 5 seconds understands how to connect their new meter to their loaner GlucoMON for automated time management and transmission to the centralized management system known as GlucoDYNAMIX.
Step 2 - Based on the predetermined settings, automated blood glucose reports are made available to any number of members of the patient's inpatient management team.
Step 3 - The patient takes the Care Kit and its contents home with them following discharge and continues to participate in the active monitoring or their blood sugar finger stick checks.
Step 4 - Patient self-care and education continue to be facilitated by the tools in the Care Kit. Provider report review becomes less frequent over time.
Step 5 - Following a typical 6 to 8 week period, the GlucoMON is retrieved either by office visit or via prepaid return mailer.
