A diabetes follow up visit conducted in the comfort and privacy of home
is now a working reality for families of children and teens with type 1
diabetes. Families only need high-speed internet access, PC or laptop,
and an inexpensive webcam to participate in this cutting edge program.
With Diabetes HouseCall, families avoid:

  1. Driving long distances to diabetes follow-up visits
  2. Missing time away from work and school
  3. Spending hours in crowded waiting rooms
  4. Seeing the diabetes specialist for just a few minutes.

After an initial visit, patient encounters are all held on line via high speed Internet connection and an inexpensive webcam.

What Kind of Patient Uses Diabetes HouseCall?

Diabetes HouseCall is not for everyone. Only families seeking the best possible diabetes care are candidates for this cutting edge approach to 21st century health care delivery. Start-up costs are minimal. The return is priceless.

Requirements

Each Diabetes HouseCall patient must meet the following eight criteria to be eligible for this program. They must:

  1. Express a strong desire to take charge of diabetes and demonstrate that attitude in their self care behaviors.
  2. Commit to this method of diabetes care for at least one year.
  3. Have access to a primary care provider to manage non-diabetes medical problems.
  4. Have high speed internet access and webcam.
  5. Use
    the GlucoMON blood glucose data system to reliably collect, analyze,
    and distribute blood sugar data to the family and doctor.
  6. Use the highly precise Homecheck A1C to obtain hemoglobin A1C results prior to each on-line visit.
  7. Make an initial face to face visit with Dr. Stephen Ponder, the developer of Diabetes Housecallto establish a proper physician-patient relationship which will then be carried over to the on-line visits.
  8. Keep
    once yearly face to face encounters with Dr. Ponder for follow up
    visits which will require diabetes-specific physical examination and
    routine laboratory studies as needed other than the hemoglobin A1C.

Expense

Diabetes HouseCall is strictly a fee-for-service program at this time. HouseCall
users are strongly encouraged to invoice their insurance carriers to
set a precedent for reimbursement of this service in the future. Some HouseCall
families have successfully obtained funding for their start-up
equipment from their flexible medical spending account. All families
are encouraged to speak directly with their insurance company's

  • The HouseCall
    tools include 4 x HomeCheck-A1c kits, Internet and video
    conferencing training, scheduling and program readiness certification.
  • The
    HouseCall service plan also includes one 30 minute education session
    that allows you be more accurate and have more confidence in your
    ability to count carbohydrates and match your own body's varying
    insulin requirements.
  • Currently available in Texas and Iowa.  
  • Provider fees are billed separately and established by the HouseCall practitioners.
