Diabetes House Call is the brainchild of Stephen
Ponder, MD CDE. Many of you know him as the Medical
Director of Texas Lions Summer Camp for Kids with
Diabetes in Kerrville. 'Dr. Steve' is also in
practice at Driscoll Children's Hospital in Corpus
Christi, Texas where, along with his multi-disciplined
diabetes staff, they care for over 550 kids with
diabetes from all across the State of Texas.
While most of your contact with Dr. Ponder will be
over the interactive video network or through the cell
phone, there are certain minimum face-time
requirements to make this relationship work. For
starters, you can only become a member of DHC after a
face-to-face visit with Dr. Ponder. After all, this
is the beginning of a long-distance relationship and
developing that relationship is key to your success.
After the initial encounter, face-to-face visits are
mandatory once every 12 months and could happen more
frequently depending on what's going on with your
diabetes. Assuming everything is rolling along with
the usual highs and lows, frequent interaction over
the DHC telemedicine network may be sufficient.
DHC Care Kits are also a regular part of the program.
Depending on your profile and the nature of the
scheduled visit, DHC staff will ship a box to your
home or work with all of the necessary tests and forms
to support the best possible upcoming visit. It
includes things like a Home A1c test kit, Blood
Pressure cuff, detailed instructions for
self-reporting glucose and insulin data along with
other medications and changes since your last visit.
Your visits will be scheduled online by the patient's
family and paid for in advance using your favorite
credit card. If you have to miss your visit, you are
welcome to grab the next vacancy available through the
online scheduling system.
If you need help getting things setup, our technical
support team is there to help you in advance of the
scheduled visit.
All that's left is for you to pick your plan; Silver,
Gold and Platinum. Or maybe you just want to try it
out one time at first before you commit to an annual
plan. DHC doesn't accept insurance. If there are
opportunities for you to file on your own, we'll help
you do it. The elimination of handling insurance
claims is one reason why it's possible to provide this
level of medical care - the way healthcare used to
work before insurance companies controlled the system.
In the months and years to come, DHC will add
additional specialist physicians and support staff as
we expand across the country including extending this
concierge-style diabetes care program to the adult
diabetes community.
