Tier 2 - Profile Based Education
All Gold level programs are available at $35/mo. as a Fee For
Service Charge. Check program details for availability and various
minimum requirements for participation.
Program participants receive a custom configured health account including their profile and various settings that they control for delivering timely and relevant information to an extended team of people. The care kit they receive following enrollment includes unique information technology that makes it easier for the patient to take care of themselves and subsequent care kits incent the patient to take an active role in their own care.
Program Fee: $35/month
Standard Co-Pay: $30/month
