Community-based personnel will coordinate patient A1c sample collection, laboratory processing, analysis and reporting including home based testing, physician office testing and traditional laboratories.In this program we're looking for all women of child-bearing age as an attempt to prevent myriad awful complications when babies are conceived by women with even slightly elevated blood sugar levels due to lack of diangosis of pre-diabetes, pre-existing diabetes and uncontrolled diabetes.

Program Fee: $50 per screened patient
